How can I create form templates?

Besides the pre-built form templates on Upvio, you can create your own form templates.

Note: Patients may fill in forms through appointment bookings, but you cannot assign forms to patients.

You can set existing forms as templates and reuse them to create new forms. To set create a new form template, please follow these steps:

1. Login to your Upvio account.
2. Go to Forms and select the form you wish to set as template.
3. Click on beside the form name.
4. On the pop-up, enable Use as template and hit Save.

Alternatively, if you're creating a template from a new form instead of from an existing one, you will find the Use as template option immediately in the Form Details pop-up.
